The applicant whose signature appears hereon applies to THE TRASH MAN, the Contractor, for the service, or services indicated, to be supplied at the address herein described, and upon request, at any other local address to which the applicant may move. The applicant understands that there is non-refundable charge of the first 2 months of services in advance. The first initial distribution of the container is at no cost to the applicant. Replacement to the original container (cart) shall be replaced at the minimum cost of $75.00 per container; and is subject to change without notice in accordance with the cost to the Contractor for purchase or such container. Applicant hereby releases, and holds the Contractor, its employees or agents, harmless from any claims or causes of action whether damages to personal property or claims for bodily injury as a result of the above described containers. The applicant acknowledges that he/she is the owner or person entitled to possession of the premises, legally liable for payment for services.
